Lusaka Apex Medical University

The Lusaka Apex Medical University Limited (LAMU) is a company incorporated in
Zambia with investments in the provision of Medical and Health Education and
other allied services within the Health Sector. LAMU wishes to invite Request
for Proposals from qualified Companies to offer Supply of service parts,
repairs and maintenance and service level agreements (One Year Period) for the
following items

LOT 1.

A. Supply of required service parts, Repair and Servicing of Generators (Cat
Olympian GEP 110-6, Cummins 30kva c 30D5P x2-77AA-G2, Wuxi Kipor Power
Generator KDE 112 STA 3, Kirloskar Green Power Koel/30kva G14K471537, Weichai
Powergen TM 68.5kva.)

LOT 2.

A. Supply of required service parts, Repair and Servicing of Trimtech Lawn
Mower

LOT 3.

A. Service and maintenance of Fire Equipment, including fire safety trainings.

LOT 4.

A. Supply of consumables, required service parts, Service and Maintenance of
Xerox Machines; Model Number D95 and D110

Suppliers to be considered for pre-qualification, applicants MUST include the
following in their applications;

> Company profile with company structure, clearly stating physical address, phone number and e-mail address
> Certificate of Incorporation or Registration
> Valid Trading License
> VAT Registration
> Tax Identification Number
> Valid and Current Income Tax Clearance Certificate
> Traceable References from three reputable organizations/companies.
> Profile and CVs for Service Providers
> Evidence of completion of businesses related to the tender (copies of completion certificates, LPOs and contracts).
> PACRA Certificate of Registration/Incorporation
> Suppliers of any Equipment must provide proof of dealership and distribution
> Company Bank Details
> Workers Compensation Fund Certification

NOTE

> Suppliers are advised to bid in no more than two categories.
> Bids in properly sealed envelopes marked “supplier pre-qualification service level agreement for provision of (nature of business)” should be dropped in the box at the reception addressed to

application shall not lead to automatic selection but shall undergo evaluation
