Supply and Delivery of Branded Masks

Habitat for Humanity Zambia has set aside funds through its donors and intends
to use the funds for the Supply and Delivery of Branded Facemasks to enhance
good hygiene practices in slum communities in Lusaka, Kabwe and Ndola to
prevent the spread of COVID19.

Habitat for Humanity Zambia is now inviting you to submit your priced
quotation for the following item

S/N DESCRIPTION QTY DELIVERY PERIOD

1 Branded Face mask with Habitat Logo in Cyan Blue using cotton material
neatly fitted with linen and a filter to aide in breathing 2500 Within 10 Days
after issue of Local Purchase Order.

2 Branded Face mask with Habitat Logo in Black using cotton material neatly
fitted with linen and a filter to aide in breathing 2500 Within 10 days after
issue of Local Purchase Order.

INSTRUCTIONS

Eligible vendors are invited to participate in this solicitation by following
the instruction below;

1\. Submit Business Registration Certificate
2\. Submit Tax Clearance Certificate
3\. Accept payment after delivery of goods
4\. Provide a sample of the face mask that will be used to compare with the
delivered goods and please note that samples will not be returned.
5\. Indicate your delivery period which should not be more than 10 days from
issue of Local Purchase Order.
6\. Your quotation should be addressed to the Procurement Officer and should
be sent via email to the address below
